Dr. Barbara Fleener and Dr. Carola Fleener are U.S. Civil Surgeons authorized to perform immigration medical examinations and fill out your I-693 Form. All ages accepted. You will receive your complete sealed medical envelope on the day of your appointment.
- Call our Clinic (941) 923-3667 and one of our friendly assistants will answer your questions and explain the process to you.
- Come to the Clinic and pick up your package with the I-693 Form and instructions.
- You will be contacted when we have received your laboratory results so you can make an appointment at our Clinic.
- Come in on the day of your appointment for your medical examination. We will complete the I-693 Form on that day and provide you with the completed package as well as a copy for yourself.
Fees:
The medical portion of this process includes a review of your medical history, review of vaccination status, evaluation of your laboratory results, medical decision-making pertaining to each applicant’s information and the physical examination.
- Total Fee, including cost of laboratory exam: $480
- Vaccines that you require are additional. You may inquire current pricing of vaccines you wish us to administer.
- Translation Services $50 additional
All fees are self-pay and payable on the day of your appointment. Insurance coverage is not accepted for immigration physical & labwork.
